Cristina is keeping a secret of her own - a pregnancy that she does not plan to go through with.

Two episodes earned a 9.5 in season two, and this one turns out to be life-threatening for everyone. After fooling around in the backyard with some homemade explosives, a man comes into the ER with a live bomb inside of his body.

Currently keeping it from exploding is a young paramedic with her arm inside of him, and soon after her handis replaced with Meredith's. It's a delicate cooperation between the surgical team and bomb squad to attempt to save everyone's lives.

It's a fight for first place this season because a whopping four episodes hold the top spot. Alexcomes into the hospital with Rebecca, announcing that she has tried to kill herself. He doesn't want to face what this means, and he claims that she does not need medication or a psych evaluation. Instead, he puts the responsibility on himself to watch her and make sure she is okay.

Meanwhile, Bailey puts Izzie in charge of the clinic as a change of pace, and Derek and Meredith are working on the end of their clinical trial with their last patient, Beth. Beth wants to go through with the surgery, even thoughit has killed every other patient, and Meredith agrees that she should. Derek is not confident about the idea.

Derek has a plan to operate on Izzie's brain tumor, but it could leave her with memory problems, as it involves taking out a portion of her brain. Izzie doesn't know whether she should go ahead with the surgery or not, but she eventually decides to have it done.

Elsewhere in the hospital, an unidentifiable man comes in who was hit by a bus as he pushed a young woman out of the way, saving her life. He can't talk, but when he reveals who he is by tracing in Meredith's hand, it turns out that the accident has hit too close to home for everyone in the hospital.

The competition to become chief resident has begun, and Meredith is determined to prove that she has what it takes to hold the position. To do this, she volunteers to run the ER for the night, hoping that her skills shine through and show all that she has to offer.

In the course of an hour, Meredith deals with a man in denial about his chest pains, a four-year-old who needs a full leg cast for his broken femur, a patient with a knife stuck in his head after a brawl, and even Chief Webber's wife Adele has come into the emergency room. Meredith certainly has her hands full.

Teddy's husband Henry needs surgery, and she insists on having Cristina do it. Owen thinks it's a bad idea, but Webber insists that they can have Cristina do the surgery without knowing who she is operating on.

Meanwhile, Meredith and Alex go to another hospital to transport a newborn back to Seattle, but when the ambulance breaks down during a terrible storm, they are sitting ducks and an accident waiting to happen, especially in a vehicle filled with oxygen tanks that could explode if they get hit.
